Sack Lunch Request Submission Process:
- Please only use the forms found in the sack lunch request links below. These are the only forms that will be accepted. Please do not download the forms to save, but rather visit the website each time to ensure the most recent version of the form is accessed.
- ONLY submit completed forms to firstname.lastname@example.org. Do no submit to your site Cafeteria Lead.
- Field trip lunch requests must be received by the Central Kitchen/Child Nutrition Services through email no later than 7 days prior to your trip. This lead time allows us to order food and ensure that we have enough staffing to meet your needs and adjust planned menu totals if needed. Requests of less than 7 days will not be filled.
- Be sure to include both teacher name AND school site on the line that reads, "Request from (Teacher/School Site)______________", so that we can process your trip correctly.
- If several classes from the same school are going on the same trip, one sack lunch request can be submitted. Please include all teachers names going on the trip on the request.
