The Program Change Form will be used for the following changes or adjustments to your Extended Day Program.
When you need to change the programs your child is enrolled in, for example changing from Before & After school (AM/PM) to just After School (PM). Please note that we allow one program change per school year at no charge, for each additional change made a $15 fee will be applied to your account.
The Extended Day Program allows for one week of vacation each school year to be credited to your account. The vacation request must be received two weeks in advance of the vacation and credit will only be given for full weeks the child is out.
--Withdrawal from the program
If you wish to withdrawal from the Extended Day Program, a two-week written notice is required. Fees will be adjusted based on the date the withdrawal request was received.
--Return to the Extended Day Program (Same School Year)
If you wish to return to the Extended Day Program in the same school year, if space is available, you do not need to complete a new application. See the program lead at the school or the district office to complete the program change form. Please note that we allow one program change per school year at no charge, for each additional change made a $15 fee will be applied to your account.
If you are ever late to pick up your child from the extended day program, you will be required to sign the program change form. Late pick up fees are charged as follows: $1.00 per minute for the first 15 minutes and $2.00 per minute thereafter.
Wait List Procedures - All Elementary Sites
Please login to the Parent Potal to complete the waitlist application. You will follow the normal process of registration, but your application will be kept on file until you are notified that you have been accepted into the program via email/or phone call.
