Field Trips Sack Lunch Request Form
To make the process easier and more accurate, all sack lunch requests must now be submitted online. PDF forms will no longer be accepted.
Deadlines
- Field Trips: Requests must be submitted at least 7 days prior to your trip.
- Requests submitted with less than 7 days’ notice cannot be filled.
Important Information
- Include teacher’s name and school site so we can process your request correctly.
- If multiple classes from the same school are attending the same trip, please submit one request form and list all teachers’ names.
Elementary Schools
- On the day of your trip, your sack lunch cooler will include a hot pink Daily Meal Count Form.
- This form must be completed in full and returned to your cafeteria lead.
Middle & High Schools
- Field Trip Lunches will be delivered to the school kitchen.
- Lunches will be available for pick up when students are dismissed to board buses.
- Students must:
- Pick up their own sack lunch from the cafeteria.
- Enter their student ID number at the serving windows.
Excessive meals that are ordered but not served will result in a $6.70 chargeback to your school per meal not served to field trip students.
